About
Our children’s palliative care service provides flexible care to meet the needs of babies and children with life-limiting illnesses due to chronic health problems or an acute medical problem or trauma. Children and young people are supported in the home or in the hospital.
We work in collaboration with GPs, children's community nurses and the Kaleidoscope team (the community based children’s palliative care team for Westminster, Kensington and Chelsea and Hammersmith & Fulham). We also work closely with the palliative care team at Great Ormond Street Hospital who will lead on symptom management and provide a 24-hour on-call service. We also work closely with children's hospices who can provide respite and support for families of children with life-limiting conditons.
Individualised care plans are made with the child and family and this may include wishes for end-of-life care. Families may decide that they wish their child to have end of life care in their own home or at a local children's hospice and this can be arranged with support for the family.
