1 Check that you live in one of our local NHS areas

We welcome self-referrals to our maternity services at Queen Charlotte’s and Chelsea Hospital and St Mary’s Hospital from individuals living in NHS North West London and NHS North Central London. That means you need to live in one of the following local authority areas: 

Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, Kensington and Chelsea, Westminster, Barnet, Camden, Enfield, Haringey or Islington.

We are sorry that we are not currently able to accept self-referrals if you live outside of these areas. We have had to limit self-referrals to ensure safe and high quality care and to maintain sufficient capacity for complex pregnancies and births in light of significant and unanticipated increases in demand, especially from outside our local areas.

We will accept referrals from GPs and other maternity units if there is a clinical need for complex pregnancy care, including maternal medicine and fetal medicine. Please speak to your GP or local maternity service. 

You can find out your options for maternity care on the NHS website. Choosing a local service will help to ensure your care is joined up, before and after you give birth.  

2 Make your referral as soon as possible

You should start antenatal care (the care leading up until the birth of your baby) early in your pregnancy, ideally within your first 10 weeks.

We will accept later referrals but we are sorry that we are not able to accept self-referrals to transfer your care from another maternity unit if you are over 30 weeks into your pregnancy, even if you live in our local NHS areas.

We will accept late referrals from GPs and other maternity units if there is a clinical need for complex pregnancy care, including maternal medicine and fetal medicine. Please speak to your GP or current maternity service.  

3 Choose how to make your referral

  • Use our online self-referral form: Complete and submit the form online.
  • Download our self-referral form: Open the PDF file and print it, complete every field, and email it to our secure address: ICHC-tr.imperialreferrals@nhs.net.
  • Call our booking team: Phone 020 3313 5000, Monday to Friday, 8 am to 5 pm. A team member will take your details over the phone.
  • Visit an antenatal clinic: Attend Queen Charlotte’s & Chelsea Hospital, Du Cane Road, London, or St Mary’s Hospital, Paddington, London, to complete your self-referral form.
  • Ask a social worker or midwife to refer you: They can submit the self-referral form on your behalf.
  • Ask your GP to make a referral: GPs have their own referral form that they can complete and email to us.

Urgent referrals

If you have not registered for maternity care anywhere and you are worried about your pregnancy or at a late stage, you can make an urgent referral. Call our maternity helpline on 020 3312 6135

Transferring your care

It’s important that you only receive maternity care from one NHS hospital at a time – to make sure you have joined up care and to help us use our resources wisely.

You can choose to transfer your care between hospitals. We accept routine self-referrals to transfer to one of our units up until 30 weeks into your pregnancy. If there is a clinical reason for you to be transferred to our maternity services after this point, your GP or existing maternity unit will make the arrangements for you.

If you are transferring to us, please let your existing maternity service know that you no longer need them. If you no longer wish to have your maternity care with us, please contact your maternity care team.
