Eligibility criteria for self-referral to our service
Before referring yourself, please ensure you meet the eligibility criteria outlined below. Meeting these criteria is essential for us to provide you with the appropriate care.
There are several ways you can self-refer to our maternity and obstetrics service to help you start your antenatal care as soon as possible

Timing of pregnancy:
- You should start antenatal care early in pregnancy, ideally within your first 10 weeks.
- If you are over 30 weeks pregnant and have been receiving antenatal care at another NHS Trust, the safest option is to remain with your current provider. If your current provider advises you to transfer, they will assist with the process.

Eligibility by location:
- Routine self-referrals after 30 weeks of pregnancy are only accepted if you live in one of the eight boroughs that make up the North West London Integrated Care System:
- Brent
- Ealing
- Hammersmith & Fulham
- Harrow
- Hillingdon
- Hounslow
- Kensington & Chelsea
- Westminster

If you meet these criteria, you can self-refer to our service in one of the following ways:
- Use our online form: Complete and submit the self-referral form online.
- Download a self-referral form: Open the PDF file, print it, complete every field, and email it to our secure address: ICHC-tr.imperialreferrals@nhs.net.
- Call our booking team: Phone 020 3313 5000, Monday to Friday, 8 am to 5 pm. A team member will take your details over the phone.
- Speak to your community midwife: They can assist with your referral.
- Visit an antenatal clinic: Attend Queen Charlotte’s & Chelsea Hospital, Du Cane Road, London, or St Mary’s Hospital, Paddington, London, to complete your self-referral form.
- Be referred by a social worker or midwife: They can submit the self-referral form on your behalf.

GP referrals:
General practitioners can download a GP referral form and email it to ICHC-tr.imperialreferrals@nhs.net.
Urgent referrals:
For urgent referrals, contact the maternity helpline at 020 3312 6135. For obstetric medicine referrals, email Mandie O'Loughin, lead midwife, at imperial.obsmedqcch@nhs.net.
