There are several ways you can self-refer to our maternity and obstetrics service to help you start your antenatal care as soon as possible.

Self-refer to our service

It is really important for you and your baby’s health that you start your antenatal care early in pregnancy – in your first 10 weeks. 

If you have been receiving antenatal care in a hospital run by another NHS Trust, and you are over 30-weeks pregnant, the safest option is to remain with your current provider. If your current provider has advised you that you need to have your care in a different hospital, they will help you with that transfer.

You can self-refer to our service in one of the following ways:

• Use our online form to self-refer to our service.
• You can download a self-referral form: When you click the link, a PDF file should open on your computer. Print it to your computer, complete every field and email your completed form to our secure email address: ICHC-tr.imperialreferrals@nhs.net.
• Call our booking team on 020 3313 5000 from Monday through Friday between 8am and 5pm and someone will take your self-referral details over the phone.
• Speak to your community midwife. 
• Come to an antenatal clinic at Queen Charlotte’s & Chelsea Hospital, Du Cane Road, London or St Mary’s Hospital, Paddington, London and complete your self-referral form there.
• Be referred to us by your social worker or by any midwife, using our self-referral form.

Please note: Currently, we are not able to accept routine self-referrals after 30-weeks of pregnancy unless you live in one of the eight boroughs that make up the North West London Integrated Care System (Brent, Ealing, Hammersmith & Fulham, Harrow, Hillingdon, Hounslow, Kensington & Chelsea and Westminster). This is due to higher than expected demand from within North West London and our commitment to maintaining high quality care. 

GPs

General practitioners wishing to refer patients to our service may download our GP referral form and send it to the email address above.

Urgent referrals

For urgent referrals, please contact the maternity helpline on 020 3312 6135. Referrals to the obstetric medicine service can be made by email to Mandie O'Loughin, lead midwife, at imperial.obsmedqcch@nhs.net.
